Thread Lift or Dermal Filler: Which One Suits Your Skin?

Both thread lifts and dermal fillers promise a more youthful, lifted appearance, which is exactly why they get confused so often. They’re not interchangeable, though. The right one depends on what’s actually happening with the skin, sagging in one case, volume loss in the other, and getting that distinction right matters more than picking whichever treatment sounds more appealing.

The Core Difference

Dermal fillers restore volume. As skin ages, it gradually loses fat, collagen, and structural support, particularly in the cheeks, temples, and around the mouth. Fillers, typically made from hyaluronic acid, fill that lost volume back in, smoothing static lines and restoring contour.

Thread lifts work differently. Fine, dissolvable threads, often with small barbs called cogs, are inserted beneath the skin to physically lift and reposition tissue that’s begun to sag. Rather than adding volume, they tighten and support skin that’s lost its structural hold, particularly along the jawline, jowls, and mid-face.

Which Concern Do You Actually Have?

This is the question that matters more than anything else. Hollow cheeks, flattened temples, or thinning lips point towards volume loss, which fillers are built to address. Jowls, a softening jawline, or skin that’s started to droop rather than simply thin out point towards sagging, which threads are designed to correct.

It’s common for both to be happening at once, particularly from the late forties onwards, which is part of why combining the two is increasingly common rather than unusual.

Speed of Results

Fillers deliver results immediately. Once any swelling settles, usually within a week, the final look is visible straight away. Thread lifts work more gradually. There’s an initial lifting effect from the threads themselves, but the fuller result develops over the following weeks as collagen builds around the insertion points.

Neither approach is better. It depends on whether instant results matter, or whether a more gradual, natural-looking change is preferred.

How Long Each Lasts

Dermal fillers typically last between six and eighteen months, depending on the product used and the area treated. Thread lifts tend to last longer, commonly twelve to eighteen months and sometimes up to two years, partly because the collagen they stimulate continues supporting the skin even after the threads themselves have dissolved.

Longevity for both varies by individual factors such as skin quality, age, and lifestyle, so these ranges are a guide rather than a guarantee.

Downtime and Recovery

Both treatments involve minimal downtime, though “minimal” doesn’t mean none. Filler swelling generally settles within a few days. Thread lifts can bring some swelling, tightness, or mild bruising in the first week, along with the occasional small dimple at an insertion point that resolves as the tissue settles. Most people return to normal routines quickly with either treatment.

Can They Be Combined?

Often, yes. A combination approach, threads to address sagging and filler to restore volume, tends to produce the most complete result when both concerns are present. Which areas need which treatment isn’t always obvious without an in-person assessment, since sagging and volume loss can look quite similar at a glance.
